If you are an existing patient of the clinic, please use the form below to let us know which supplements you need to refill. We will process payment and contact you to schedule a pick-up date. If we don’t have any in stock, we will contact you with an option for our online dispensary or let you know when the item is back in stock. Please note to use our supplement refill feature, your most recent appointment has to be within the last 6 months.
